It's a clever cartoon because it combines several references to recent events and ties them together- Obama's smoking habit, the girl at Obama's budget speech, and the facts that quitting smoking is hard because it's an addiction just as our politicians can't quit spending because it's addicting for them.

Then there's clever pictoral depiction of 'fiscal responsibility' going up in smoke.

IOW, the cartoonist did what cartoonists do, and very well- he combined ideas into a coherent picture that draws on things we're all familiar with to make a point.
